Lodging the application
You should lodge the Application for Expedited Assessment - Form 1, plus supporting documentation, with the Commission. You can download Form 1 or call us for a copy on 1300 368 040.
If you need help completing this form, please read our Guide to completing Form 1.
You must also send a copy of your application to the employer and insurer on the same day as you lodge the application with us.
We may obtain the insurer's views about your application before deciding whether to issue the Interim Payment Direction.
